The higher-level work

• The ALJ hearing request (OMHA-100) filled, not just described, at level 3.
• The Medicare Appeals Council review request letter at level 4, citing 42 CFR Part 405, Subpart I's own hearing and Council timelines.
• The good-cause package when a deadline was missed for a real reason — a letter, not a bare form, built from your client's own facts.
• Medicare Appeals Council decisions in the underlying knowledge corpus, cited by letters where they apply — not just the regulation text.

The DAB-101 request for Council review of an ALJ decision is referenced, with a link to CMS's own form; CaseWhy Appeals doesn't fill it yet.
