Free one-page NOMNC and Medicare Advantage denial deadline cheat sheet
For SNF, home health and hospice teams. One page, printable, written against the regulations and the CMS notice text — the dates your staff need when a notice lands and nobody is sure what the clock is.
- Deliver the NOMNC at least two calendar days before covered services end. If care is not given daily, deliver it by the second-to-last day of service. It is two calendar days, not 48 hours.
- The resident's fast appeal deadline: "as soon as possible, but no later than noon of the day before the effective date" on the notice. The request goes to the QIO by phone or in writing.
- Standard reconsideration: ask the plan within 65 calendar days of the date on the denial notice. The rule gives 60 days from when you receive it, and receipt is presumed 5 days after the notice's own date — 60 + 5 = 65 from the date printed on it.

General information, not legal advice. The cheat sheet summarises CMS's own NOMNC instructions and the Medicare Advantage appeal regulations; it is not a substitute for reading the notice in front of you or for advice about a particular resident.
If a notice is already in front of you, CaseWhy Appeals reads it, shows the exact deadline, drafts the appeal letter citing the rule the plan must follow, and tracks every case on one board. Your first 3 cases are free.
